Please note that it is the player's responsibility to submit the completed form, which should be accompanied with:

1) A copy of the rejection/denial from the player's own medical insurance company for the treatment
2) A copy of the medical insurance card of the player's own insurance
3) All bills and receipts from the hospital along with the medical report
4) The passport, visa and Emirates ID of the player
